Program Request Form
Please fill out the following information to let us know what type of program you are interested in booking and other important details. Once we receive this form, a member of our staff will reach out to you to confirm the availability of our educators, discuss the best schedule and share information related to materials needed for the program and issue your invoice. Once you have completed the form if you have any further questions or concerns, please feel free to reach out by email at hello@archforkids.com.

What date would you like to start the series? (Please note requests must be made at least one week in advance and we cannot guarantee your preferred start date will be available)
Please list the 4 days and times you would ideally have your series take place. We will do our best to accommodate this but based on educator availability may need to select different dates or times.
Scheduling: Afterschool Availability
In the case that we are unable to accommodate your preferred dates it would help to have a general idea of which days your student is most likely to be available. Please check which afternoons (after 3:30PM) are most likely to be open on the student's calendar.
